We have a dedicated Chronic Care management team . We have partenered with Passionate Care Chronic Care. The nurses will call eleigible patients every month to go over refills, health concerns etc. Eligible patients will also get a BP monitor synchronised to Passionate Care CCM, so thatw e can monitor your home BP readings. Please ask about the service at your next appointment.
